What to have ready
A clear photo or PDF of each one, with the dates showing. You can photograph them with your phone as you go.
- Fingerprint clearance card
- Your Arizona DPS Level 1 card, front side with your name and expiration date.
- CPR
- Your current card or certificate.
- First aid
- Your current card or certificate.
- Food handler card
- Your county food handler card.
- TB screening
- Your most recent skin test or chest X-ray result.
- Certificates and training
- Caregiver or med tech certificates, and any training you already have.

- Join and choose what to shareYou decide whether the home sees your credential dates and your documents. You can change it any time.
- Set up your profileYour name as it appears on your fingerprint card, your phone and your role.
- Add each credentialTake a photo or upload a file, then enter the dates. CareCrew reminds you before anything expires.
- We verify themThe office checks each document against your staff record. Once everything is verified you are ready for the schedule.
